How many sessions are allowed?
Each client is permitted up to six sessions per contract year 

                      
If I use all of my allowable sessions, can I come back?
Yes.  As long as you remain employed with the company, you will have six sessions for your use each contract year.



How much does it cost?
EAP services are free for employees and family members who reside with them.



Will my employer know if seek out services?
If you seek out services voluntarily your employer will not know of your involvement.  However, if you are referred mandatory; the Lifespan therapist will ask you to sign a release of information to communicate general compliance matters with the referring party.

Do I have to use all six sessions?
No.  The primary objective of your EAP therapist is to assess your needs and determine a clinical course of action.  This is often done in less than six sessions leaving your remaining sessions available for future use (if needed) within the contract year.



If I don’t use any or all allowable sessions, do they carry over to the next contract year?
No. However, you will six new sessions each calendar year whether you use them or not. The allowable number from year to year will not exceed six unless the Lifespan contract with your employer is amended.  



What if I need more session beyond the allowable six?
Your Lifespan therapist will work closely with you to insure that proper linkage to the necessary services is completed prior to discontinuing services. 



Can I stay with my EAP therapist for more ongoing services beyond the EAP?
If the EAP therapist involved with you feels that they are a good clinical fit for the issues needing attention, this is definitely an arrangement that can be considered.



If I continue to work with my EAP therapist beyond the scope of the six EAP sessions are the services still free?
No.  Your therapist will work with you to confirm insurance benefits (if applicable) and you will be responsible to abide by the guidelines of your specific mental health coverage.  If this is not an option, you would have to pay a rate of $90 per clinical hour.  Depending on your financial situation, a sliding scale fee might be an option. 



Can I use Lifespan if I am no longer employed with my company?
No.  However, Lifespan does have contracts with many other Lake County businesses so you would be eligible for our services if you obtained employment with a contracted company.  



If I do not attend a session do I have to pay for it?
If a session is missed and the Lifespan therapist feels that the missed appointment was negligent on the client’s behalf, that session is deducted from the total allowed that contract year. 


Can more than one family member be involved in EAP services at the same time?
Yes.  Lifespan therapists often see employees and their family members if family therapy is the identified need.  However, different family members can also be using the EAP service at the same time, but have different therapists.  



When are appointments held?
The Lifespan therapists work to accommodate your schedule.  Sessions are held at all hours of the day from 9:00AM until 8:00PM, Monday through Thursday, and from 9:00AM to 5:00PM on Fridays.  There is limited availability on Saturdays. 



What if I need to speak to my therapist after business hours?
Lifespan therapists provide their clients with a voicemail number that has crisis prompts for any after urgent matters that arise after business hours.  For life threatening emergencies clients are asked to call 9-1-1 or to go to their nearest emergency room.
